We are a 4-Star skilled nursing facility ranking higher than any facility in Daviess County along with one of the highest ranking Medicare and/or Medicaid certified facilities in the State of Indiana.
The scores are derived from the results of the annual Indiana State Department of Health annual licensure and recertification process. In 2011, the facility attained the highest survey results possible with no state or federal deficiencies cited.
